This case features a white woman in her 40s who came to Dr. Katzen frustrated by loose skin and fullness along her upper waist, sides, and back after weight changes. She wanted a more defined torso and understood that no amount of diet or exercise could tighten stretched, redundant skin. After examination and discussion of options, Dr. Katzen recommended a Vertical Torsoplasty, an upper body lift, and body contouring surgery to remove excess skin and reshape the side chest and flank regions. In the operating room, she was placed under general anesthesia. Dr. Katzen marked a vertical incision along the side of the torso, extending from beneath the breast toward the hip. Through this incision, he elevated the skin and underlying fat from the muscle layer to expose the areas of redundancy. He removed extra skin and localized fat from the side chest, lateral roll region, and upper waist, then advanced the remaining tissues together to tighten and narrow the torso. By lifting and re-draping the tissue upward and inward, he created a smoother transition from the back to the front of the trunk and enhanced the waistline contour. Internal sutures were placed to support the new shape, and the incision was closed in multiple layers to reduce tension and encourage healing.
