Our patient is a 28-year-old White male who came to Dr. Timothy Katzen with buttock discomfort and pain after silicone injections performed by another surgeon. Because free silicone can migrate, trigger inflammation, and create firm pockets, Dr. Katzen recommended a closed silicone injection removal using an ultrasound-guided, VASER-assisted method, a silicone removal technique designed to break up and evacuate silicone through small access points rather than large open incisions when the distribution is appropriate. Under general anesthesia, Dr. Katzen first assessed the affected regions to plan the safest and most effective site for removal. He then created tiny, strategically placed openings and used VASER ultrasound energy to gently emulsify the silicone and reactive fluid, helping separate unwanted material from healthier tissue. A specialized cannula was used to suction out the loosened silicone in a controlled, step-by-step pattern, reducing the risk of unevenness. Throughout the procedure, Dr. Katzen focuses on protecting the skin, avoiding over-aggressive removal, and maintaining smooth contours. Post-operative care includes compression and walking to promote circulation, with follow-up to monitor swelling. These photos were taken one week after surgery, and while firmness is still expected at this stage, he already looks remarkably improved and reported feeling relieved from most of his pain as healing continues and inflammation settles.

VASER Closed Silicone Removal

Our patient is a 28-year-old White male who weighed 180 lbs at the time of surgery, he came to Dr. Timothy Katzen with ongoing discomfort and pain in the buttock region caused by previous silicone injections performed by another surgeon.  After a thorough evaluation, Dr. Katzen recommended a closed silicone injection removal using an ultrasound-guided, VASER-assisted method, a specialized technique designed to target unwanted silicone while minimizing large incisions and helping protect surrounding healthy tissue.  Under general anesthesia, Dr. Katzen carefully planned small, discreet access points and introduced VASER ultrasound energy to gently loosen and break up the silicone and inflammatory fluid that can form around it.  This step helps separate the unwanted material from the body’s natural tissue, making removal more controlled and reducing the risk of uneven contours.  Once the silicone was mobilized, a specialized suction cannula was used to gradually extract the loosened material in a methodical pattern, focusing on relieving pressure, improving comfort, and maintaining a smoother buttock shape.  Throughout the procedure, Dr. Katzen prioritized safety, tissue preservation, and balanced body contouring, which are critical in complex silicone removal cases.  These photos were taken one week after surgery, and while some swelling and firmness are still expected at this early stage, our patient reported remarkable improvement and significant relief from most of his pain, with continued healing and refinement anticipated as inflammation continues to settle over the following weeks.
