Our patient is a Black male, age 26, who came to Dr. Timothy Katzen after silicone injections four years earlier began causing pain and discomfort in the buttock region. Because migrated silicone can spread into multiple pockets and trigger ongoing inflammation, Dr. Katzen recommended a Posterior Butt Lift with silicone injection removal using the open injection removal method. Under general anesthesia, Dr. Katzen made strategically placed incisions to directly access the areas where silicone had collected and migrated, allowing controlled removal of silicone deposits and irritated scar tissue while protecting healthier structures. Our patient is a Black male, age 26 at the time of surgery, who presented with pain and discomfort after prior silicone injections to the buttocks performed by another surgeon four years earlier.  Dr. Timothy Katzen recommended a Posterior Butt Lift with open injection removal because migrated silicone can create firm, inflamed areas that are difficult to treat without direct access.  Under general anesthesia, Dr. Katzen used an incision based approach to locate the most symptomatic silicone pockets, carefully remove silicone deposits and surrounding scar tissue, and preserve healthy tissue whenever possible.  This open injection removal method provides greater control than blind removal techniques when silicone has traveled beyond the original injection sites.  After reducing the silicone burden, Dr. Katzen refined the posterior buttock contour to improve shape, symmetry, and support, creating a smoother appearance that fits the patient’s frame.  Early recovery includes swelling, bruising, and firmness, with gradual softening over time as healing progresses.  Photos were taken four years after surgery, showing stable, long-term results.  Our patient achieved remarkable improvement and has shared that he is relieved from most of his pain, with better comfort in sitting and daily life.
