Migrated Silicone Removal with Posterior Butt Lift for Smoother Female Contour*

After years of living with migrated foreign body injections, our patient wanted lasting relief and a safer, smoother contour.  She is a 39-year-old Hispanic female who weighed 150 lbs at the time of surgery and came to Dr. Timothy Katzen with ongoing pain and discomfort in the buttock region because the injected material had spread into surrounding tissues.  Dr. Katzen recommended an open silicone injection removal with a posterior butt lift to remove the problem tissue directly and rebuild the posterior shape in the same operation.  Under general anesthesia, Dr. Katzen made a planned posterior incision to access the involved layers, then carefully identified areas of silicone-laden and inflamed tissue that were contributing to tenderness and pressure.  Using an open approach allows direct visualization, which is important when material has migrated because it can sit in multiple pockets and create firm, painful areas.  Dr. Katzen removed the foreign material and affected tissue in a controlled, step-by-step manner, focusing on protecting the skin’s blood supply and preserving healthier tissue whenever possible.  After removal, he performed the posterior butt lift by tightening deeper support layers, removing loosened skin, and lifting the posterior tissues to create a smoother transition from the lower back into the buttocks.  The incision was closed in multiple layers to reduce tension and support long-term healing, and postoperative compression and follow-up helped manage swelling and fluid.  These photos were taken 3.5 years after surgery, showing her remarkable long-term results with improved contour and lasting comfort, and she shared that she is happy to be relieved from most of her pain.
