Silicone injection removal is often necessary when illegal buttock injections lead to pain, deformity, and migration of material into nearby tissues. Our patient is a 31-year-old White female who came to Dr. J. Timothy Katzen after receiving foreign body injections to the buttocks in Las Vegas in 2017. Over time, she developed discomfort and skin changes and became concerned that some of the product had spread toward the groin, also called the inguinal region. This can happen because liquid silicone does not always stay in one place. Instead, it may move through tissue planes, trigger inflammation, and form dense scar tissue that changes the shape and feel of the buttocks. Under general anesthesia, Dr. Katzen used the open injection removal technique, which means the affected area is surgically opened so the silicone-contaminated tissue can be seen and removed directly. This is important because illegal silicone often becomes embedded within the tissue and cannot simply be suctioned out. During surgery, the goal is not only to remove visible silicone but also to remove unhealthy tissue altered by the foreign material. After removing the diseased tissue and scar tissue, Dr. Katzen performed a posterior butt lift to lift and reshape the buttocks, helping restore smoother contour and better symmetry for a more natural result today now. One of the main reasons illegal silicone injections are so dangerous is that the material can spread, harden, and damage otherwise healthy tissue.   Our patient is a 31-year-old White female who saw Dr. J. Timothy Katzen after receiving foreign body injections to the buttocks in Las Vegas in 2017.   She later developed pain, skin deformity, and concern that some of the silicone had migrated toward the groin, known medically as the inguinal region.   In cases like this, buttock silicone removal is not simply about taking out one lump.  The silicone may be scattered through the soft tissue, surrounded by scar tissue, and responsible for chronic inflammation.  That is why Dr. Katzen performed an open injection removal technique under general anesthesia.  By directly opening the area, he could identify and remove tissue affected by the illegal silicone injections, rather than leaving contaminated tissue behind.  Once the damaged tissue was removed, Dr. Katzen performed a posterior butt lift to elevate the buttocks and improve contour after the excision. This reconstructive step is important because removing silicone can leave the area loose, uneven, or deflated if it is not reshaped.
