After losing a significant amount of weight, our patient wanted a body contouring plan that would not only reduce stubborn fullness but also rebuild areas that looked flat after weight loss. He is a 44-year-old White male who previously struggled with morbid obesity, reaching a maximum weight of 315 pounds before lowering to 165 pounds through diet and exercise. Dr. Timothy Katzen included a circumferential lower body lift, fat transfer to the buttocks in his surgical transformation plan, using fat removed by torso liposuction to naturally enhance buttock shape and projection. Under general anesthesia, Dr. Katzen first performed liposuction of the torso region to sculpt the waist and surrounding areas while harvesting usable donor fat. That fat was then carefully cleaned and purified to isolate the healthiest fat cells for transfer. The focus of this procedure was the buttock fat transfer, in which Dr. Katzen injected the processed fat into carefully selected areas of the buttocks to improve fullness, refine the contour, and create a more lifted, balanced appearance. Butt Augmentation Fat Transfer

Our patient is a 44-year-old White male who had previously been morbidly obese, with a maximum weight of 315 lbs before losing a significant amount of weight through diet and exercise and reaching 165 lbs.  After working so hard to transform his body, he came to Dr. Timothy Katzen looking for a full-body transformation that would better match his new frame and improve areas that still lacked shape and proportion.  One important part of that plan was a 360 circumferential lower body lift, fat transfer to the buttocks after liposuction of the torso region, with special focus on creating a stronger, more balanced buttock contour using his own fat.  Under general anesthesia, Dr. Katzen first performed liposuction of the torso to remove unwanted fat from areas where fullness was disrupting the patient’s contour.  Liposuction is not just about removing fat, it is also about strategically collecting donor fat that can be reused for body sculpting.  The harvested fat was carefully collected and then purified so only healthy, usable fat cells would be selected for transfer.  This purification step is extremely important because it helps improve the quality of the fat graft and supports a smoother, more natural result.  The highlight of this surgery was the buttock fat transfer.  After preparing the fat, Dr. Katzen injected it into the buttocks using a careful, layered technique.  Rather than placing the fat in one single area, he transferred it in multiple passes and in different zones of the buttocks to improve projection, restore shape, and create a fuller, more athletic contour.  This type of buttock augmentation with fat transfer is designed to use the patient’s own tissue to enhance volume in a way that looks and feels natural.  It also allows Dr. Katzen to customize the shape, helping build better balance between the waist, hips, and buttocks after major weight loss.  By combining torso liposuction with gluteal fat grafting, Dr. Katzen was able to slim one area while adding volume to another, which is one of the reasons this technique can create such a dramatic improvement in overall silhouette.  The goal was not simply a larger buttock, but a more proportional, smoother, and stronger lower-body contour that fit the patient’s transformed body.  These photos were taken 2 months after surgery, and our patient achieved remarkable results. He is already showing improved buttock shape, better projection, and a more sculpted torso, with continued refinement expected as swelling continues to decrease and the transferred fat settles.
