This is a white 28-year-old female patient who came all the way from Miami to have a 360 circumferential lower body lift with Dr. Katzen. Her maximum weight was 400 pounds but with gastric bypass surgery, she was able to get down to lose 215 pounds. With massive weight loss, you’re bound to have excess skin. This patient particularly didn’t like the excess skin and fat over her hips, tummy, buttocks, and mons region. Dr. Katzen performed a 360 circumferential lower body lift to address all of these areas. Her weight was 185 lbs on the day of surgery. The surgery lasted seven hours under general anesthesia and Dr. Katzen was able to remove 16 pounds of skin and fat. Her postop photos were taken four months after surgery. This patient is recovering well and is happy with her new body, tight abdomen, and ability to move and exercise freely. The procedure can also address the relationship between the lower abdomen and mons pubis when indicated. Dr. J. Timothy Katzen, Beverly Hills plastic surgeon, evaluates patients seeking tummy tuck surgery after gastric bypass and other major weight-loss journeys. This case emphasizes individualized planning, attention to umbilical appearance, and lower abdominal and pubic contouring. A medical consultation is necessary to determine candidacy, discuss potential complications, and establish realistic expectations for recovery and results.

Post-Weight Loss Body Lift

Before-and-after photographs of a 28-year-old woman, 5'7" and 185 pounds, who lost 130 pounds after reaching a maximum weight of 315 pounds. Her weight-loss journey included Roux-en-Y gastric bypass (RNY), diet, and exercise. Following massive weight loss, she underwent abdominal body contouring to address excess skin and improve the appearance of her abdomen and pubic region. Her postoperative results highlight a flatter abdomen, a natural-looking belly button, and a raised, reduced pubic region. An important feature of this case is that the desired contour was achieved without a vertical fleur-de-lis (FDL) incision. Fleur-de-lis abdominoplasty is sometimes necessary when patients have significant excess skin in both the horizontal and vertical directions. However, when the pattern of skin laxity permits, a traditional horizontal tummy tuck incision may provide substantial improvement without an additional vertical abdominal scar. Surgical planning also considers umbilical positioning and the relationship between the lower abdomen and mons pubis to create a balanced contour. Dr. J. Timothy Katzen, Beverly Hills plastic surgeon, evaluates patients seeking tummy tuck surgery and post-bariatric body contouring after gastric bypass and significant weight loss. This case demonstrates the importance of customizing the operation to the patient's anatomy rather than automatically choosing a more extensive incision pattern. Patients considering abdominoplasty after massive weight loss should discuss incision placement, permanent scars, recovery, potential complications, and realistic expectations during a personalized consultation.
