Our patient is a 44-year-old White male who achieved an extraordinary weight loss transformation, reducing his weight from a maximum of 315 pounds down to 165 pounds through dedication to diet and exercise. While this accomplishment dramatically improved his health, it also left him with excess skin hanging from his back, a common concern for patients after massive weight loss. Seeking a full-body transformation to better match his new physique, he came to Dr. Timothy Katzen to address the loose tissue that could not be improved through lifestyle changes alone. As part of his body contouring plan, Dr. Katzen performed a back lift, also known as a torsoplasty, using a vertical incision to remove the redundant skin and reshape the contour of the back. Under general anesthesia, Dr. Katzen carefully marked the areas of greatest skin laxity while the patient was standing, which helps identify where the skin hangs and how it should be tightened for the most natural result. During the torsoplasty procedure, vertical incisions was made along the sides to provide direct access to the excess tissue. This approach allows the surgeon to remove the stretched skin and underlying tissue in a controlled and precise manner, improving the overall shape of the back while maintaining natural proportions. After the excess skin was removed, the remaining skin was lifted and re-draped to create a smoother and firmer contour. Dr. Katzen then closed the incision in multiple layers to reduce tension on the skin and support optimal healing. A vertical torsoplasty is often recommended for patients who have experienced massive weight loss and have excess skin that hangs primarily from the back rather than around the entire waist. By removing this redundant tissue, the procedure can significantly improve how clothing fits and restore a more defined body silhouette. These photos were taken two months after surgery, and our patient achieved remarkable results. His back now appears noticeably smoother and more contoured, helping his body better reflect the dedication and effort he invested in losing a significant amount of weight.

Our patient is a 44-year-old White male who worked incredibly hard to transform his body after previously being morbidly obese, reaching a maximum weight of 315 lbs before lowering it to 165 lbs through diet and exercise. Even after this major accomplishment, he was still left with excess skin hanging from his back, a common concern after massive weight loss that can make clothing fit poorly and hide the progress underneath. As part of his full-body transformation, Dr. Timothy Katzen performed a back lift, also known as a torsoplasty, using a vertical incision to remove the loose, hanging tissue and improve the overall contour of the upper and mid back. Under general anesthesia, Dr. Katzen carefully marked the area of greatest skin excess, then created a vertical incision that provided direct access to the redundant skin on the back and underlying tissue. The excess tissue was removed in a controlled and even manner so the back could be tightened and reshaped without creating an unnatural appearance. The remaining skin was then re-draped and carefully closed in multiple layers to support healing and reduce tension on the incision. This type of post-weight-loss body contouring is designed to create a smoother, firmer back profile and help the body better reflect the weight loss already achieved. These photos were taken 2 months after surgery, and his results are already remarkable, showing a tighter back contour, less hanging skin, and a more complete transformation.
