J Timothy K: | All right, so basically she is a small B and she wants to go up to a triple D. And to achieve that we need to put implants in and we're going to use 800 CC implants. So first of all, let's dissect our pocket and get the expander in there and see what she can achieve with a expander in place. Now if you want the light off, I can do that. |

J Timothy K: | I'm dissecting through the breast tissue now and I'm going down to [inaudible] pocket and then I'll create a pocket for the tissue expander and we'll expand that out. And then once that looks good, then we'll put it in our implant. So I'm just basically going through the breast tissue very carefully, making sure all the bleeding is controlled and we don't make our pocket too big or too small. So I dissected out the pocket and it looks like a good pocket. So we're going to put in our tissue expander now and expand the pocket to make sure it's adequate for our implant. All right, so this is an expander. Basically it's an empty balloon and we're going to inject air or some people use saline, and we're going to pump it up. So I put in one. So we're going to put the expander in. |
That's our pocket. So I've injected two, three, four, five, six, seven, eight, nine, ten, eleven, twelve, thirteen, fourteen, fifteen. | |

J Timothy K: | Sixteen. Yes, that's 800 right? |

J Timothy K: | Because I put 50. Yup. Perfect. So there you go. So you can see before and after, before, after. So this is with the augmentation, this is without. Just to be clear, no implant, augmentation. Just so there's no confusion. |

So basically I'm going to repeat the same steps for the side. I'm going to inject my local here to minimize any bleeding and I'm going to make my incision. Make a little pocket for the implant, put the tissue expander in, and then we'll sit her up, make sure everything looks good and put it in our permanent implants. So there's the expander, we're going to put it in and this is just temporary, and it helps dissect the tissue out and make sure everything looks good. So very important, we put the right amount in here. So one, two, four, five, six, seven, eight, nine, ten, eleven, twelve, thirteen, fourteen, fifteen. |

J Timothy K: | Yeah, sixteen right? Yeah. So it's very important that the implants do not touch. So I want to dissect this close but I want to make sure that they have two separate pockets because if they communicate then you get this uni-breast and that's a horrible deformity. So you want to make sure there's a little bit of a gap. Maybe about two centimeters. The width of my thumb right in between there. And then with the type of bra she wears, she can get as much cleavage as she want. All right, so 800s, looks awesome. So now we go with the implants. |
Yeah, I think the syringe. Oh, that's smart. Genius. Genius. [inaudible 00:05:44]. So this is our implant. A nice 800 CC silicon implant and it's like a sphere that's cut in half, so it does have a defined shape. So let's put this in its new home. | |
There we go. Good. I think that looks really good. And that's a 800 CC implant, and I think it fits her body very well. There we go. So I call that 0 to 860 seconds. Sorry. In 60 minutes. Yep. And if you look at her face, she's smiling already. You can just tell. Stapler. Let's sit her up now. | |
Ah geez. That one needs to go down. So now I'm making sure that the height from this point, the top of the sternum here is equal on both sides. So I do this thing called tik tok where I take the suture and I measure from the this point to this point and make sure this is perfectly equal on both sides. So this is a little bit, a couple of millimeters lower on this side. So I'm going to have to bring up this side just a couple of millimeters to get it equal. Going to do a little bit of a lift on this side, and the other sides are good to go. Yeah. I think I'm going to cry. They're so beautiful. Who is this? | |
