Thigh skin laxity can be especially frustrating after weight loss because it may remain visible and uncomfortable even after a patient reaches a lower weight. Our patient is a 37-year-old Hispanic female who had previously been morbidly obese, with a highest weight of 320 pounds. After diet, exercise, and RNY gastric bypass, she reduced her weight to 212 pounds but continued to struggle with excess skin hanging from her thighs. Dr. Katzen performed a medial thigh lift, also called an MTL, as part of her comprehensive body transformation plan while she was under general anesthesia. During surgery, Dr. Katzen removed loose inner thigh skin and excess tissue to improve leg contour and reduce the sagging appearance caused by stretched skin. A post-weight-loss medial thigh lift is not meant to replace weight loss; it is a body contouring procedure that removes skin that no longer responds to diet, exercise, or continued weight reduction. Her photos were taken 4 months after surgery, and she achieved remarkable results. Our patient’s thighs now look smoother, cleaner, and better balanced as part of her lower-body transformation. This case highlights how thigh lift surgery after gastric bypass can refine the inner thighs, improve leg proportion, and help patients feel more comfortable in clothing. For patients after massive weight loss, an MTL can be an important final step in treating loose skin and completing the overall body contour.

A complete lower-body transformation often requires addressing the thighs after the abdomen, hips, and lower body have changed with major weight loss. Our patient is a 37-year-old Hispanic female who had previously been morbidly obese and reached a highest weight of 320 pounds. Through diet, exercise, and RNY gastric bypass, she reduced her weight to 212 pounds, but she was still bothered by excess skin hanging from her thighs. As part of her comprehensive body transformation plan, Dr. Katzen performed a medial thigh lift, also known as an MTL, while she was under general anesthesia. During this inner thigh lift procedure, Dr. Katzen removed loose skin and extra tissue from the medial thighs to improve sagging and create a smoother leg contour. A medial thigh lift after weight loss is designed for patients whose skin has stretched and cannot tighten enough with exercise alone. This procedure can help reduce hanging thigh skin, improve the shape of the upper legs, and create better balance with the rest of the lower body. Her photos were taken 4 months after surgery, and she achieved remarkable results. Our patient’s thighs appear smoother, tighter, and more proportional after her post-bariatric body contouring procedure. This case shows how thigh lift surgery can help complete a weight-loss journey after gastric bypass by removing excess skin that remains even after significant weight loss.
