Our patient is a 56-year-old Middle Eastern female who weighed 139 lbs at the time of surgery. She came to Dr. Timothy Katzen 2 years after her original breast augmentation. Over time, some patients can develop changes around an implant, including a firm or tight capsule of scar tissue that forms naturally around the implant as part of the body’s healing response. When that capsule becomes thick, tight, or uncomfortable, or when it begins to distort breast shape, a capsulectomy can be recommended to remove the problematic scar tissue and help restore a softer, more natural contour. In this case, Dr. Katzen performed a right breast capsulectomy as a specialized breast implant revision procedure to improve the feel, shape, and overall symmetry. Under general anesthesia, Dr. Katzen reopened the prior incision as discreetly as possible to access the implant pocket while minimizing additional scarring. He then carefully identified the capsule, which is the internal layer of scar tissue surrounding the implant. A capsulectomy involves separating that capsule from the surrounding breast tissue and removing it in a controlled, meticulous manner. This step is important because it addresses the source of tightness and contour irregularity rather than simply adjusting the implant alone.
