A softer, more natural breast shape is often possible after implant revision surgery when painful capsular contracture is properly treated. Our patient is a 44-year-old Asian female who previously had bilateral breast augmentation and later developed bilateral painful breasts because of capsular contracture around her saline implants. She came to Dr. J. Timothy Katzen seeking the removal of the bilateral saline breast implants and bilateral capsulectomies and replacing them with silicone implants. Under general anesthesia, Dr. Katzen performed this breast revision procedure by removing the old implants, surgically excising the scar tissue capsules surrounding them, and placing new silicone breast implants to improve comfort, contour, and softness. With breast implant surgeries, capsular contracture happens when the natural scar tissue around an implant becomes tight and constricting, which can cause pain, firmness, and distortion. A capsulectomy is performed to remove that problematic scar tissue so the breasts can be reconstructed in a healthier way. Silicone implants were chosen to provide a smoother, more natural-looking, and natural-feeling result. Her photos were taken 1 month after surgery and already show remarkable early improvement in breast shape, symmetry, and softness. She is also relieved of most of her pain, making her outcome a beautiful example of how revision breast augmentation can improve both function and appearance.
