Our patient is a 36-year-old Hispanic female who came to Dr. Timothy Katzen with pain and discomfort after foreign body injections were placed in the buttocks by another surgeon. She also believed the product had migrated to the left leg, which can occur when injected material spreads through tissue planes and forms inflamed, firm pockets that feel tender and tight. Dr. Katzen recommended an open silicone injection removal with a posterior butt lift to directly remove the affected tissue and rebuild a smoother, safer contour. Under general anesthesia, he made a carefully planned posterior incision to access the involved layers, then identified the injected material and surrounding inflamed tissue that was contributing to symptoms. Using an open approach allows direct visualization rather than blind suction, which is critical when the material is irregular, scarred, or has migrated. Dr. Katzen removed silicone-laden and reactive tissue in a controlled, step-by-step manner, focusing on symptom relief while preserving healthy tissue and the skin’s blood supply. After removal, he performed the posterior butt lift by tightening deeper support layers and removing loose skin to improve shape and smooth the transition from the lower back into the buttocks. The incision was closed in multiple layers to reduce tension and support better scar healing, and postoperative compression with close follow-up helped manage swelling and fluid as tissues settled.

Migrated Silicone Butt Removal

Our patient is a 36-year-old Hispanic female who weighed 207 lbs at the time of surgery and came to Dr. Timothy Katzen with ongoing buttock discomfort, persistent pain, and concerns that previously injected foreign body material had migrated toward her left leg.  After evaluation, Dr. Katzen recommended an open silicone injection removal with posterior butt lift, a reconstructive body contouring approach designed to directly remove inflamed, silicone-laden tissue and then reshape the posterior contour in the same operation.  Under general anesthesia, Dr. Katzen used a carefully planned posterior incision to access the affected tissue planes, identify pockets of foreign material, and separate unhealthy tissue from healthier structures.  The open technique is especially important in complex migration cases because it allows direct visualization rather than blind removal, improving precision and helping protect blood supply to the skin.  After methodical removal of symptomatic foreign body material and irritated tissue, Dr. Katzen irrigated the area, controlled bleeding, and proceeded with the posterior butt lift portion by tightening deeper support layers, trimming excess tissue, and re-draping skin to create a smoother, more balanced buttock profile.  The incision was closed in multiple layers to reduce tension and support stronger healing, and postoperative care included compression, activity guidance, and close follow up to monitor swelling and contour maturation.  These photos were taken 6 months after surgery, and her results are remarkable, with improved posterior shape, better symmetry, and meaningful relief from the discomfort and pain that previously affected her daily life.
