Dr. Katzen:

This is a little local medication, just to numb up this area before we make an incision to harvest this fat, which then we're going to put in the butt.

Okay, so now I'm going to make an incision for our port site.

Now we're going to put in our tumescent solution.

I want to put in about a liter, here. And then we'll suck out that fluid, of a liter, and some fat for harvesting

This is a little skin port, and I'll show you exactly what it is, here. We had the hole in the skin and one of the problems with a hole in the skin when we do liposuction, the liposuction port, the liposuction cannula; if this is the skin with the hole, there's a lot of friction in the skin, and it can cause a really bad scar. So what we do, is we put this little port in here, which is this little protector. It's this little protector, and it protects the skin when the port goes in and out. So it prevents that port from rubbing up against the skin. And it's an awesome instrument. Look at that. It's titanium. It weighs nothing. It's super lightweight. It's what? About $5,000? Almost.

Speaker 2:

Chump change.

Dr. Katzen:

Chump change, yeah. This is a titanium surgical instrument. Let's see where it's made. Oh, it's made in the USA. Yay! Good. But beautiful instrument, titanium. About $5,000. All right, let's go double double. Yep.

So now, the lidocaine, epinephrine; it's all working its magic in here. It's numbing up the area, it's making sure that fat is all good and ready for the harvest. And now, we're going to go harvesting.

So here's our MicroAire Power-Assisted Liposuction machine. I turn it on, the tip vibrates. It's got two ports. Should be able to harvest fat pretty well.

So look at the fat, as it comes out. It'll be nice, golden yellow. See? Beautiful fat per [inaudible].

So on this side, we got out about 1,500 CCs. So, pretty good fat. 1,500 CCs. We got out about 1,300. So all this is really good fat to harvest and put in the buttock.

It's a good feature; a good safety feature. But sometimes too much is too much. My car has nothing. Naked.

So we're done with the liposuction. She has a nice, good curve here; nice, good hipbones. Nice curvature of the body. I'll put the fat that we harvested from here into the buttock. And you can see in here we have a really good harvest. This is almost three liters. And we have one liter up to almost three liters. So this is about two liters of pure fat that we can inject into the buttock. So it's high-quality fat. So I think she'll be really happy with that.

Speaker 2:

So this is our fat, here. We've already strained it and we've gotten rid of all the tumescent solution, which helps prevent blood loss; and we washed it with an antibiotic irrigation. And once we reconstitute it, then we can load it up into syringes.

Dr. Katzen:

So I'm going to make a small incision underneath the buttock, here. And that allows access for our fat to be transplanted. So we're going to squeeze that through there, and it's going to come out through here, the tip, and we're just going to lay it down like planting rice or grass or whatever you want.

Okay, go ahead.

Speaker 2:

5, 10, 15... 25, 30... 50, 55, 60.

Dr. Katzen:

Ten.

Speaker 2:

40, 45... 10, 15, 20.

Dr. Katzen:

So I break it down into thirds; an upper third, middle third, lower third. So I put three syringes up here, two syringes here. We got one more to go for this one. Uh-huh (affirmative).

Speaker 2:

5, 10... 40...

Dr. Katzen:

Inconspicuous incision underneath the buttock. And so now we got top third, middle third, bottom third. Okay, go ahead.

So he's injecting it through this tube, and I'm moving the cannula back, and we're planting this fat like strips, like this. Okay, go ahead.

Speaker 2:

Five...

Dr. Katzen:

There's a special cushion I have everybody wear after the fat transfer. We measure the patient, we measure their buttock. This is an open space where they sit, and so their buttock rests in here, so there's no pressure on the buttock. So they'll rest in here for about two weeks, and it takes all the pressure off. So we're going to slide her in here, now.

Without even thinking, I know about five.
